The following link will open a new window containing the ATF Form 4473 for Over-the-counter Transaction Record. This form is being provided on-line in order to expedite the NCIC Background Check and Transaction evolution. Use of this form is NOT required, as you may wait and accept a printed copy of form 4473 when you arrive to accomplish your transfer.

This form contains TWO pages of data fields and TWO pages of Instructions.

If you are able to print TWO-SIDED, you may print the pages of data on one sheet.

TO accomplish a firearm Transfer, you will need to fill in every applicable answer in Section A ONLY and Certify with your signature on item 14 BEFORE presenting to FFL/Transfer Agent.

You may also enter specific applicable information required into items Numbered 16 through 18c, and 21.

HOWEVER, you MUST present the original documentation or cards for personal verification by Transfer Agent before a Transfer can occur.

 On the Top-Right of the form, you may select "HIGHLIGHT FIELDS" to display fields that will require your attention.
Please be sure to OVERWRITE words marking these fields, such as: NAME, ADDRESS, etc.
